The nominations for the sports year 2025 will be available on December 1, 2025.

Submission Deadline — January 31, 2026

Sport Awards Night: Friday, April 10, 2026

• Nominee(s) for an individual award must be a resident of Welland; nominees for a team or club award must be Welland based.
• Separate Forms must be submitted for each nomination
• Any person can nominate individuals or teams in one or all categories ~ Exception: Executive*
• Grassroots Coaching is career or lifetime achievement
• Check off selection (one per form only)
• See Appendix for Policy and Procedure information

Please provide the following information — restrict accomplishments to the reflect the year 2025. Exception: The Grassroots Coach which is career or lifetime achievement.

IMPORTANT: Attach photocopies of newspaper articles, pictures, etc. to support your nomination.
Materials become the property of the Sports Promotion Committee and will not be returned.
