Vital Statistics:

BIRTH, DEATH, OR MARRIAGE CERTIFICATE Applications for events in the Province of Ontario can be obtained from the Clerk’s Department at City Hall or on the ServiceOntario website at www.serviceontario.ca.

Applications for all other Provinces and Territories in Canada are also available for in-person pick up at City Hall or on the ServiceCanada website at www.servicecanada.gc.ca.

The Province of Ontario is in the process of rolling out a new Integrated Birth Registration (IBR) Program designed to make the registration of a newborn’s vital birth information substantially faster to enable documents such as Birth Certificates, Social Insurance Numbers, Passports, etc. to be processed in a more timely and secure manner.

The City of Welland is proud to be one of the first municipalities in Ontario to be a part of this roll out.  Other municipalities will be included in the months and years to come.

At this time, Welland is somewhat unique to other municipalities not presently part of the IBR process.  Therefore, as a caution to users of this website, the procedures for birth registration for babies born in Welland may differ from processes in other municipalities in Ontario that are not yet part of the IBR Program.

Any child born at the Niagara Health System - Welland Site (Welland County General Hospital) or in the City of Welland through a midwife service or other means after November 18, 2007, must register through the IBR process with the Province of Ontario.  A child born in the last year, before November 19, 2007, must still register through the City of Welland, Clerk’s Division.

Do I have to register my new baby?

Yes.  The birth of every child born in the Province of Ontario must be registered within 30 days of the date of birth with the Office of the Registrar General if the birth occurred after November 18, 2007, or, with the City of Welland if the birth occurred before November 19, 2007

The Statement of Live Birth is your child’s permanent legal record and allows documents such as Birth Certificates, Social Insurance Numbers, and Passports to be produced.  The birth registration form itself cannot be used as proof of identity.

How do I register my new baby?

In Welland, for a child born after November 18, 2007

In Welland, for a child born after November 18, 2007, parent(s) will submit birth information either electronically or in paper format directly to the Office of the Registrar General for processing.   Parents are encouraged to use the electronic Newborn Registration Service. 

To submit electronic birth information, please access the ServiceOntario website at www.serviceontario.ca/newborn.  If choosing the electronic format, you may also complete on-line applications for a Birth Certificate and Social Insurance Number at the same time; this will make it easier and faster to obtain these documents.  The Office of the Registrar General states that parents taking advantage of this 3-in-1 program can expect to receive their baby’s Birth Certificate in as little as seven (7) days.  If both parents are going to be named on the birth registration, then both parents must be present at the computer during the process. 

If you are unable to or do not wish to electronically submit your baby’s birth information, parent(s) may opt to use the paper form (fill out the form ensuring the information is clear and accurate).  Paper forms will still be available at the Hospital or Municipal Office for parents who prefer to submit documents by mail.  Once you have completed and carefully reviewed the Statement of Live Birth, the parent(s) who are named on the Statement must sign the form as indicated.  Parent(s) will forward the paper form directly to the Office of the Registrar General at PO Box 4600, 189 Red River Road, Thunder Bay, ON  P7B 6L8.   Please be aware that this process takes considerably longer than the online process.

The Office of the Registrar General does not charge a fee for the registration of birth information; although, fees do apply for various other documents including Birth Certificates.

How do I register my new baby?

In Welland, for a child born before November 19, 2007

In Welland, for a child born before November 19, 2007, parent(s) will submit birth information directly to the City of Welland for processing.  The municipality will forward the form to the Office of the Registrar General to register the birth with the Province.  You should have received a Statement of Live Birth form from the Niagara Health System - Welland Site (Welland County General Hospital) or midwife.  If you have not received a Statement of Live Birth form, you can obtain one in person at the City of Welland, Clerk’s Division.

The Statement of Live Birth contains the particulars of your Baby’s birth.  Please fill out the form ensuring the information is clear and accurate (this will be your child’s permanent legal record).  Once you have completed and carefully reviewed the Statement of Live Birth, the parent(s) who are named on the Statement must sign the form as indicated.  The Statement may be signed by the mother alone only if the father’s/other parent’s information is not provided on the form.  Please refer to the Important Instructions attached to the Statement of Live Birth form when completing the Statement.   These instructions provide necessary details for completing the form, as well as important instructions on naming your child, etc.  Parent(s) will forward the paper form to the City of Welland, Clerk’s Division, 60 East Main Street, Welland, ON  L3B 3X4.

What does Father’s/Other Parent’s Information mean?

A “Father”, for the purposes of the Statement of Live Birth form, must be the biological father of the child and consent to be acknowledged as the father.  An “Other Parent”, for the purposes of this form, must be another person who consents to be acknowledged as the parent, if the biological father is unknown and the child was born of assisted conception with an anonymous sperm donor.  The father’s/other parent’s information may be included on the form, if the mother acknowledges that person as a parent of the child.  If the father’s/other parent’s information is on the form, that parent must also sign the form.

What if the child’s father/other parent refuses to provide their information?

If the father’s/other parent’s information is on the form, that parent must also sign the form.  If the father/other parent refuses to provide their information, then their information cannot be included on the Statement of Live Birth form.

What surname (last name) can I give my child?

If both parents’ information is included on the Statement of Live Birth form, the child’s surname may be either parents’ surname or former surname, or both parents’ surnames or both parents’ former surnames hyphenated or combined.  If only the mother’s information is included on the form, the child’s surname may be the mother’s surname of former surname.  The parent(s) who sign the form may also give the child a surname based on the child’s ethnic, religious, or cultural heritage.  If this option is chosen, the parent(s) must check the appropriate box in section E.

What if we cannot agree on the selection of our child’s surname?

If “No” is checked in Section E indicating that both parents do not agree on the child’s surname (last name), or, if this section is left incomplete, the registered surname will be the parents’ surname if they have the same surname.  If the parents have different surnames, the child’s surname will be registered using both parents’ surnames hyphenated in alphabetical order.

There is no fee to register birth information using the Integrated Birth System; however, the Province does charge a fee to process a Birth Certificate.

Is there a fee to register my new baby?

Births that occurred in Welland prior to November 19, 2007

Births that occurred in Welland prior to November 19, 2007 will be subject to a City of Welland Administrative Fee of $25.00.  The only form of payment accepted is:  certified cheque, cash, debit, or money orderPLEASE NOTE:  Personal cheques cannot be accepted and will be returned to you; therefore, delaying the registration process until the fee is received in one of the acceptable forms noted above.

PLEASE NOTE:  The City of Welland Administrative Fee for babies born before November 19, 2007, does not provide you with a Birth Certificate.

How do I obtain a Birth Certificate?

During the Online Integrated Birth Registration (IBR) process

When you register your baby’s birth using the online IBR process you will be prompted during the process as to whether you wish to apply for a Birth Certificate at the same time.  If you choose “yes”, you child’s Birth Certificate will be processed as well as the registration.  Once registration is complete your child’s Birth Certificate application will be processed immediately and you can expect to receive it in as little as seven (7) days.

Online application (for births that have already been registered)

If you choose not to apply for Birth Certificate during the online IBR process, application for Birth Certificate can be made online at a later date at www.serviceontario.ca.  There are service options available online such as:  Regular Online Service, 15 business days (including delivery); and, Premium Online Service, 5 business days (including delivery).  PLEASE NOTE:  Service times only apply to births that have already been registered.  Currently, the fee for a first time Birth Certificate (not replacement) is $25.00 for a short form (wallet-size) certificate, and $35.00 for a long form (certified copy of birth registration).   If applying for documents through the Premium Online Service, a $30.00 surcharge for each document will apply.

Manual application (for births that have already been registered)

If you choose not to apply for a Birth Certificate online, you may apply for a Birth Certificate manually.  Forms for a manual application are available for in-person pick up at City Hall or on the ServiceOntario website at www.serviceontario.ca.  The Birth Certificate application should be completed and forwarded directly to the Office of the Registrar General at PO Box 4600, 189 Red River Road, Thunder Bay, ON  P7B 6L8, for processing.  Currently the fee for a first time Birth Certificate (not replacement) is $25.00 for a short form (wallet-size) certificate and $35.00 for a long form (certified copy of birth registration).   If you require faster service than 6-8 weeks, please apply online or contact the Office of the Registrar General at 1-800-461-2156 or in Toronto at 416-325-8305.  Expedited Service takes 10 business days plus delivery time and you must provide Proof of Urgency*.  Emergency Service is 2 business days plus delivery time and Proof of Urgency* is required (these services are only available at the Toronto Office.  A $30.00 surcharge for each document will be charged).

*Proof of Urgency may include:

  • A medical emergency
  • A letter confirming new employment
  • A letter from a consulate or embassy confirming an appointment
  • Airline tickets
  • A letter confirming an appointment for an immigration hearing
  • Travel reservations
  • A wedding invitation, where the applicant is the bride or groom

Other situations are subject to review.

How do I obtain a Travel Letter?

Travel Letters are no longer available for births occurring on or after November 19, 2007.  If parents are required to travel with their newborn and have not applied for a Birth Certificate at the time of registration, please contact the Office of the Registrar General at 1-800-461-2156 or visit www.serviceontario.ca.

Parents are encouraged to complete the registration process and apply for a Birth Certificate on-line, at which time births are registered electronically and Birth Certificates issued immediately following completion of electronic registration.
